What about oral estradiol?

Estrofem, including Estrofem 2 mg, is among the oral estradiol products found in the UAE. Oral estradiol can control menopausal symptoms, but its pharmacological profile differs from the transdermal route because it passes through the liver. For women with obesity, migraine, liver disease, increased thrombotic risk or significant metabolic risk factors, clinicians therefore give particular consideration to whether a transdermal option is appropriate.

Why does the endometrium need protection?

When the uterus is present, systemic estrogen requires a progestogen to protect the endometrium. Utrogestan is one familiar option in Dubai and contains micronized progesterone. UAE pharmacies carry 100 mg and 200 mg presentations.

Micronized progesterone is structurally identical to progesterone produced by the human body. It is therefore often part of what is described as a body-identical or bioidentical HRT regimen.

Femoston: a combined formulation

Femoston 1/10 and Femoston 2/10 are widely represented on the UAE market. They combine estradiol with dydrogesterone and can be convenient when a fixed oral regimen suits the patient.

Dydrogesterone is a synthetic progestogen that has been studied in menopausal hormone therapy and is used for endometrial protection. For some women, a combined formulation is simpler than separate estrogen and progesterone products.

Duphaston and body-identical progesterone

Duphaston 10 mg is also available in Dubai. Its active ingredient is dydrogesterone, which can be used as a separate progestogen alongside estrogen for endometrial protection and is included in recommendations as an option.

When the aim is specifically a body-identical regimen, micronized progesterone such as Utrogestan is generally preferred because its molecule is identical to natural human progesterone. That does not make Duphaston an unsuitable product: it is a different option selected according to indications, tolerability and the individual regimen. The British Menopause Society also describes a favorable profile for dydrogesterone in relation to endometrial protection and predictable bleeding patterns.

Livial

Livial 2.5 mg, containing tibolone, is another product available in the UAE. It is a synthetic steroid with estrogenic, progestogenic and androgenic activity, rather than a conventional estrogen-plus-progesterone combination.

It may be used for some postmenopausal women, but has its own indications, limitations and contraindications. It is not a universal substitute for other forms of menopausal hormone therapy.

When may local estrogen be enough?

Systemic HRT is not always necessary when the main symptoms are vaginal dryness, burning, discomfort during sex, urinary urgency or other features of genitourinary syndrome of menopause. Vagifem is a vaginal estradiol product available in the UAE.

Local treatment using a small amount of estrogen acts mainly on vaginal and urinary tissues. Its purpose differs from systemic therapy used, for example, to control hot flushes.

What does bioidentical HRT mean?

Bioidentical hormones have the same molecular structure as hormones produced by the human body. Estradiol and micronized progesterone fall into this category. However, the term does not automatically mean safer or more suitable for every woman. International societies recommend registered, standardized medicines with a known composition and controlled active ingredient content over individually mixed compounded hormones when licensed alternatives are available.
